In 2022, we established SM Health with a clear vision and purpose. Sadia Mansoor, PharmD, BS, AAS
Sadia is the founder of SM Health Inc. She has a rich educational background and diverse professional experience that uniquely shape her role in the healthcare industry. In 2016, she earned her Doctor of Pharmacy degree from Touro College of Pharmacy, building upon a solid foundation with a Bachelor of Science in Biology from Stony Brook University. Additionally, Sadia holds an Associate of Applied Science in Medical Laboratory Technology degree from Nassau Community College.
Before embarking on her journey as a pharmacist, Sadia worked as a Medical Laboratory Technician in community physician offices, gaining invaluable insights into the healthcare landscape. Upon obtaining her PharmD degree, she ventured into the community pharmacy setting, where she honed her skills. Recognizing the transformative impact of Medication Therapy Management (MTM) services, Sadia transitioned into contracting with Pharmacy Benefit Managers (PBMs) to provide specialized MTM services.
Driven by a passion for improving patient care, Sadia founded SM Health. Her vision for the company revolves around complementing the existing workflows of community practices by offering services such as Chronic Care Management (CCM) and Remote Patient Monitoring (RPM). With a commitment to innovation and enhancing healthcare delivery, Sadia's leadership propels SM Health towards its mission of providing comprehensive and patient-centric solutions to healthcare practitioners and their communities.

Mohammad S. Ahsan PharmD, MBA
Mohammad, a results-oriented pharmacist, brings over 11 years of dedicated experience in the institutional setting and boasts a remarkable 14 years of combined expertise within the broader healthcare landscape. Graduating magna cum laude, he earned his Doctor of Pharmacy degree from Long Island University in 2012.
For the past 11 years, Mohammad has been an integral part of a prominent health system, where he navigated diverse roles, showcasing his versatility and commitment to patient care. His journey included serving as a clinical pharmacist with a focus on overseeing pharmacy services in cardiology and geriatrics. In 2023, Mohammad achieved another milestone by graduating summa cum laude with an MBA in Healthcare Administration from SUNY Oswego.
Eager to embrace innovation, Mohammad transitioned into an information technology role within the health system. Here, he was tasked with the implementation of a new Electronic Health Record (EHR) system, showcasing his adaptability and forward-thinking approach to healthcare management.
Currently, as a key figure at SM Health Inc., he manages day-to-day operations, steering marketing initiatives, and overseeing the financial aspects of the organization. His wealth of experience and diverse skill set position him as a driving force behind SM Health's commitment to providing top-notch healthcare services.
